Course Dates: March 21- June 10, 2005


APPLICATION DEADLINE : February 21, 2005
Non-refundable Application Fee of $50.00 Required 
Non-refundable Acceptance Fee of $500.00 Required within 1 week of acceptance. 
(If admitted to program Application Fee & Acceptance Fee is applied to tuition payment)

TUITION: $2,300.00 $1000.00 payment due: March 4, 2005
Remaining tuition of $750.00 will be due March 21, 2005 (first day of program)
No refunds will be given upon withdrawal or if you choose not to attend the course. 

Schedule for 12 week course:
Week I : Monday-Thursday
Week II: Monday-Friday
Week III: Monday-Thursday
Week IV-Week XII: 3 days/week in clinical

PHYSICAL: Required prior to class; Solaris Health Forms must be taken to your private physician for completion. These forms must be returned to:
